Enter a few basic pieces of information and wait for your confirmation email/text . When we receive your appointment request, we will send a confirmation and everything you need for your first appointment.
Fill out your new patient paperwork, submit it online, and wait for your appointment. Reminders and information about your appointment will be send via email or text.
Immediately before your appointment, use your phone, tablet or computer to click on the telehealth link sent to your phone or email. Log on from anywhere in Florida and meet with the same friendly nurse practitioner every time. Prescriptions are sent directly to your preferred pharmacy.
When your appointment is over, payment is collected online : $175 for initial psych eval, and $90 for follow up appointments via flex, HSA and credit/debit card. Superbills are available upon request for patients to submit to their insurance for reimbursement.
